Adenoiditis and Otitis Media with Effusion: Recent Physio-pathological and Terapeutic Acquisition

چکیده
OME, middle ear disease with many denominations, is a subacute or chronic inflammation by multifactorial etiology, characterized by the presence of serous, mucous, or mixed, non-infected secretion, persistent for more than three months in the middle ear. The incidence is higher in children (ratio children / adults = 10/1), particularly pre-school (61%) than school age (22%), occurs mainly in winter and spring coinciding with infectious and/or allergic inflammation of Upper Airway and Digestive Tract (U.A.D.T.). The incidence of OME after acute otitis media (AOM) is high, with a percentage between 6-10% for some authors or even 42%-60% for others authors; the persistence of secretion in the tympanic chest after AOM is almost considered normal and is attributed mainly to the presence of immunecomplexes. Inflammation of the middle ear and tubal dysfunctionare the most important etiopathogenic factors: were identified a number of established risk factors (infection of upper airway-UA, adenoid hypertrophy, seasonality, inadequate antibiotic therapy, genetic and racial factors) and risk factors accessories (familiarity allergic, mild immunodeficiency, sex, geographical and environmental factors). The role of upper airway inflammation in the onset of middle ear inflammation is documented in the most recent data highlight the importance of bacterial and viral infection, allergy, immuneinflammation and tubal dysfunction. Environmental Risk Factors for Otitis Media with Effusion in Schoolaged Children in Tehran
SUMMARY 2000 ,Tehran schoolaged children, were included in a randomized trial to evaluate the risk factors for otitis media with effusion through interviews, a written questionnaire, otoscopy pneumatic and tympanometry. We found that the incidence of OME increased in children with parents who smoke (P<0.001) and were bottle fed as babies (P<0.005). Others factor such as, allergy, sinusitis, p...
متن کاملPolymerase Chain Reaction, Bacteriologic Detection and Antibiogram of Bacteria Isolated from Otitis Media with Effusion in Children, Shiraz, Iran
Background: Otitis media with effusion is one of the leading causes of hearing loss in children. Effective treatment of effusion in the middle ear requires appropriate empirical treatment and characterization of responsible pathogens. Objective of the present study was to detect pathogens in clinical samples from patients with otitis media with effusion in our area and to determine the sensitiv...
متن کاملComparison of Adenoidectomy and Myringot-omy with and without Tube Placement in the Short Term Hearing Status of Children with Otitis Media with Effusion: A Preliminary Re-port
Background: Otitis media with effusion (OME) is the most frequent indication for surgery in children. Some surgeons prefer to use adenoidectomy and myringotomy with tympanostomy tubes insertion to prevent the sequelae of the disease and also more improvement of hearing. The aim of this study is to compare adenoidectomy-myringotomy with adenoidectomy-myringotomy plus tympanostomy tube in the man...
متن کاملOtitis Media with Effusion in Children and the Impact of Risk Factors on Serum Cytokine Levels
Introduction:To evaluate the role of allergic-type and infectious-type cytokines in children with chronic otitis media with effusion (OME) Materials and Methods:We investigated serum levels of interleukins (IL)-4, IL-5, and IL-13, along with interferon-gamma (IFN-γ) and tumor necrosis factor-alpha (TNF-α), by enzyme-linked immunosorbent assay (ELISA) in 35 children with OME and 28 healthy contr...
متن کاملThe Effects of the Early-Onset Otitis Media on Cognitive Skills in Children: A Systematic Review
Background and Objectives: Some children are at risk of middle ear infections; however, the impact of the fluctuating hearing sensations on cognitive systems and immature central auditory is not fully recognized. Therefore, we reviewed and discussed the impact of early-otitis media on the cognitive skills of affected school-aged children. Methods: A computerized search of the databa...
